Measuring meaningful change and readiness for education
Back on Track Pathways focuses on supporting pupils to move from disengagement and distress towards confidence, stability, and readiness for learning. Progress is measured through meaningful changes in emotional wellbeing, engagement, independence, and ability to access education.
The aim is not simply attendance at sessions, but sustainable improvement that enables pupils to move forward positively.
Emotional & Behavioural Development
• Reduced anxiety related to learning or school
• Improved emotional regulation
• Increased confidence and self-esteem
• Greater resilience when facing challenges
• Improved ability to manage transitions

Functional Skills Development
• Increased confidence with maths, reading, and writing
• Improved problem-solving and communication skills
• Ability to apply skills in real-life contexts
• Progress towards recognised outcomes where appropriate

Readiness for Reintegration
• Increased tolerance for structured learning environments
• Reduced avoidance behaviours
• Greater confidence about returning to education
• Preparation for transition to school or alternative placement

• Improved concentration and persistence
• Rebuilding positive attitudes towards education

Independence & Personal Development

Monitoring Progress
Progress is reviewed regularly to ensure provision remains purposeful and aligned with agreed goals.
Monitoring may include:
• Observation of engagement and participation
• Review of emotional wellbeing and confidence
• Tracking of functional skill development
• Feedback from the pupil
• Communication with parents and referring professionals
This information can support ongoing planning and decision-making.
Alignment with EHCP Outcomes
Where pupils have an Education, Health and Care Plan (EHCP), support is designed to contribute towards relevant outcomes, particularly those relating to emotional regulation, engagement in learning, independence, and communication.
Working Towards Positive Next Steps
Back on Track Pathways aims to support pupils to progress towards:
• Successful reintegration into school
• Increased attendance or participation
• Transition to a suitable educational placement
• Engagement with training or other appropriate pathways
Provision is intended to be a stepping stone towards sustainable engagement with education, not a long-term substitute.
Communication with Referrers
Feedback can be provided to schools, local authorities, or other professionals to support review meetings and future planning.
